What is an IHP and when should we create one?

Individual healthcare plans (IHPs) are intended to support pupils with physical and mental health conditions to manage their conditions. They lay out what needs to be done, when and by whom. They should be confidential, but easily accessible to everyone who needs to refer to them.

Decide on a case-by-case basis whether you need to create a plan

Consult with the pupil, their family, and the pupil’s GP or paediatrician about the child’s needs. Decide together whether a plan is needed to manage the condition.

If you can't reach consensus, the headteacher should make the final decision.
