Use this template before a pupil who uses crutches starts or returns to school. The person responsible for health and safety in your school (usually the headteacher) should fill it in.
You'll want to:
- Consult:
- The pupil and their parents
- Any staff members who may be affected, including teachers and site staff
- Adapt it based on the specific circumstances of your school
Make sure you have a personal emergency evacuation plan (PEEP) to go alongside this assessment.
