Use and adapt our risk assessment templates to address potential hazards for pupils in wheelchairs or who use crutches. Be clear on the hazards you need to consider and see example risk assessments.

Download our template risk assessment
Use this template before a pupil who uses a wheelchair starts or returns to school. The person responsible for health and safety in your school (usually the headteacher) should fill it in.
You'll want to:
- Consult:
- The pupil and their parents
- Any staff members who may be affected, including teachers and site staff
- Adapt it based on the specific circumstances of your school
You might also need to consult the wheelchair manufacturer, and the health professional in charge of the pupil's wheelchair provision, for further guidance on potential risks and how the wheelchair can be controlled.
