KeyDoc: teacher appraisal report template (maintained) DOC, 211.5 KB

It reflects the The Education (School Teachers’ Appraisal) (England) Regulations 2012 which state that you must:

  • Review the performance of all teachers in your school for each appraisal period (usually every 12 months) – this applies to all school teachers, including those who are members of the senior leadership team (SLT)
  • Provide a written report to teachers for that appraisal period

Our template is also in line with the joint NEU and NAHT recommendations for appraisal.

Adapt our template to follow your school's appraisal policy.

Using the template for senior leaders

When adapting the template for members of the SLT, you should include:

  • An objective on leadership
  • Some reference to how the objectives link to the wider school and the school's improvement plan
