Planning for transition

Support for pupils with special educational needs (SEN) should include planning and preparing for the transition between phases of education.

To support this, you should:

  • Share information with the school, college or other setting the pupil is moving to
  • Agree with parents and pupils the information to be shared as part of this planning process

If the pupil is staying at their new school for post-16 provision, you should also consider how to provide a high-quality post-16 study programme.
