- The actions you're taking as part of your investigation
- The disclosure (the initial conversation when an allegation was disclosed)
- Safeguarding actions
- Evidence collected, including witness statements
- The investigation outcome(s)

The Advisory, Conciliatory and Arbitration Service (Acas) has a template report that includes space to record the process and how evidence was collected, as well as areas for the facts established and any mitigating circumstances.
