Forms for recording and reporting violent incidents

HSE example
The Health and Safety Executive (HSE) has a violent incident report form.
It has space to indicate whether the incident being reported involved:
- Verbal abuse/threats
- Theft
- Physical attack
- Anti-social behaviour
- A near miss
It also asks:
What action has been taken to ensure that this type of incident does not reoccur?
A section at the end of the form is for a manager to record whether the incident needed to be reported to the HSE under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR) and, if so, the date when this was followed up.
