Examples

Primary school

 

Holly Primary School in Nottinghamshire has a Violent Incident Register in Appendix 3 (page 26) of its Violence Against Staff policy.

It includes details of the incident and the outcome, including details of any follow-up support offered.

Local authorities 

Check if your LA has any specific requirements or templates for violent incident reporting.

Derby City Council has an Abuse, aggression and violence (AAV) Report Form on it's website. 

Schools are required to report AAV incidents within 7 days.
