Download our template risk assessment
Use this template to complete a risk assessment before a pupil who uses a wheelchair starts or returns to school. The person responsible for health and safety in your school (usually the headteacher) should fill it in.
You'll want to:
- Consult:
- The pupil and their parents/carers
- Any staff members who may be affected, including teachers and site staff
- Adapt it based on the specific circumstances of your school
Make sure you have:
- A personal emergency evacuation plan (PEEP) to go alongside this assessment
- A separate manual handling risk assessment if staff will need to lift or carry the pupil or wheelchair – see our list of mandatory risk assessments and our staff briefing on manual handling
For further advice on potential risks and how the wheelchair can be controlled, contact:
- The wheelchair manufacturer
- The health professional in charge of the pupil's wheelchair provision
