In this article, we provide guidance and templates to help you create a 'school-based' special educational needs (SEN) support plan. The DfE recommends using school-based plans to support pupils in the special educational needs and disability (SEND) Code of Practice (paragraph 6.48).

To create your plan, you'll need to have first identified the provision the pupil needs and worked out its cost. You can use our guidance on calculating the cost of provision to help you. 

Decide on the contents of the plan based on its purpose 

There's no set format or requirements for what you need to cover, so it's up to you to decide based on what would be most useful for the individual pupil. Below you'll find a range of templates that cater for the different purposes of the plan.
