Use a consistent format for SEN support plans

Check with your local authority (LA) to find out if it requires a specific format, or use our template to develop your own records.

Use the template to record:

  • Pupil details and relevant data
  • SEN support tier
  • Identified areas of need and specific barriers
  • Adjustments to provision, targeted interventions and external specialist support
  • Pupil targets and progress 
  • Review notes and status updates

This plan tracks the 4 parts of the ‘assess-plan-do-review’ cycle outlined in the graduated approach, and can be used as evidence for future education, health and care (EHC) needs assessments.

For pupils with an EHC plan, the SEN support plan can act as a day-to-day working version that connects the requirements to specific school provision, and breaks down outcomes into smaller, more manageable targets.

Your school must formally notify
